2.) Enter zip code and search radius
3.) Search by name or category

2.) Enter your zip code.
3.)Choose Doctor by Type, Doctor by Name, or Health Facilities.
4.)Enter search parameters or choose from the dropdown menu and click Search.
5.) Select Continue as Guest
6.) Select Continue.
7.) Select, PPO
8.) Choose from list of results

2.) Highlight PHCS (Logo on front card) and click Select and Search
3.) Enter your search parameters, your zip code, and select Search.
Plan Types
PPO Copay Plans
Enjoy the freedom of a full PPO with predictable copays. Choose between the nationwide Aetna, PHCS, or Cigna PPO networks, with no referrals needed to see a specialist. Most office, specialist and urgent care visits are covered with a low copay, and you still have coverage if you see an out-of-network provider. Preventive care and many generic prescriptions are covered at no cost, and unlimited telemedicine visits are included. Choose from several low deductible options to fit your budget.
Humana Dental
Keep your smile healthy with Humana Dental PPO coverage. See any dentist you choose, with the most savings when you stay in Humana's PPO network. Preventive care like exams, cleanings and x-rays is covered at 100% in network with no deductible. Basic services like fillings and extractions, and major services like crowns, root canals, bridges and dentures, are covered too. Choose a plan with an unlimited annual maximum and orthodontia coverage for adults and children, or a lower-cost plan with a generous annual maximum.
Health VL Plans
An affordable option for everyday care. VL plans use the PHCS PPO network and keep costs simple, with low copays for office and urgent care visits, preventive care and wellness visits at no cost, and unlimited telemedicine included. Prescriptions and maternity care are covered, too. VL plans include set limits on visits and hospital stays, so they're a great fit for healthy individuals and families who want solid coverage at a lower cost. Choose from several low deductible options.
Humana Vision
See clearly for less with Humana Vision. Get a yearly eye exam for a low copay, plus a generous allowance toward frames or contact lenses every 12 months. Standard lenses are covered for a low copay, and you'll receive discounts on lens upgrades and extra glasses at network providers. Members with diabetes also receive additional eye care and testing at no cost. Out-of-network benefits are available, too.
HSA-Compatible Plans
Keep your premium low and save for health costs tax-free. Our HSA-qualified plans pair a higher deductible with a Health Savings Account, so you can set aside pre-tax money for medical expenses and keep what you don't use. Choose between the Aetna, PHCS or Cigna PPO networks. Plans include no annual benefit maximum, in- and out-of-network coverage, and preventive care at no cost. Several deductible options are available.
